Proactive Care Pilots in Barking and Dagenham: Shaping Holistic, Coordinated Care




Evaluating the Proactive Care Pilots, an ambitious initiative led by local Primary Care Networks and schools to transform how health and care services work together.
We have been selected as the evaluation partner for the Proactive Care Pilots in Barking and Dagenham, an ambitious initiative funded and driven by local Primary Care Networks (PCNs) and schools. This project introduces innovative care models across three key areas:
- Adult Proactive Care: Focused on adults identified through national criteria, offering holistic assessments and personalised care plans.
- Asthma Care: Supporting children and young people with complex asthma,
reducing reliance on emergency care. - SEND Proactive Care: Targeting children with special educational needs and
disabilities in a selected local school to provide coordinated support.
Evaluating Impact and Shaping Future Care
Our role as evaluators includes:
- Process Evaluation: Tracking how patients are identified, consented and supported through their care journeys, leveraging adapted PRISMA methodology
- Outcomes Analysis: Capturing both subjective and objective measures of success, including patient satisfaction, health improvements, and reduced hospital admissions
- Collaborative Development: Co-designing databases and tools with care coordinators to ensure seamless tracking and scalability.
Why it matters
These pilots aim to transform how health and care services work together by integrating multidisciplinary teams (MDTs) to provide tailored support. By addressing complex needs holistically, the pilots aspire to improve patient outcomes, enhance staff collaboration, and build a replicable framework for proactive care across the borough.
