Seasons Greetings from Care City
Reflecting on a Year of Impact: Collaborating for Healthier, More Connected Communities in 2025.
This year has been one of incredible collaboration, growth and learning for Care City. Together with our partners and community members, we’ve delivered impactful projects and initiatives across health, care and community services in North East London (and beyond), reflecting our shared commitment to making a real difference.
Highlights from 2024:
1. Strengthening Home Healthcare
We partnered with local authorities, GP practices and care providers in North East London and Devon to enhance home healthcare. By training care workers to use remote monitoring technology, we enabled earlier detection of illness and faster escalation to healthcare professionals, helping individuals stay healthier and more comfortable at home.
2. Empowering Independence Through Technology
With the CARE Adult Social Care Technology Fund, we are collaborating with the London Boroughs of Redbridge and Barking and Dagenham to integrate technology into care. In Redbridge, we have partnered with the Falls Team, Reablement Service and care homes to reduce falls. In Barking and Dagenham, our evaluation of care technologies aims to improve older adults’ independence and care efficiency.
3. Supporting Allied Health Professionals (AHPs)
Our work mapped AHP roles across Barking, Havering and Redbridge to address workforce challenges, inequalities and recruitment barriers. A key achievement was a cross-sector work experience programme for 25 young people, inspiring the next generation of AHPs and now being independently scaled by system partners.
4. Reducing Health Inequalities with Community Chest
We evaluated the Community Chest programme, which brings together NHS, local authorities and the VCSE sector to co-create funding initiatives for unmet community needs. Our findings, shared at a London-wide workshop run alongside Transformation Partners in Health & Care, showcased how collaboration fosters trust, integration and impactful change.
From storytelling workshops to initiatives combating social isolation, we are co-designing solutions alongside residents, empowering them to rebuild connections and drive change.
6. Championing Dementia-Friendly Spaces
Through our work with the Dorothy app, we helped make community spaces more accessible for people living with dementia, promoting inclusion and independence through technology.
A community-driven initiative funded by the Cadent Foundation addressed fuel poverty in Barking & Dagenham. By distributing resources like fuel vouchers and hosting Winter Support Community events, we improved health, resilience, and social connections for vulnerable residents.
8. Growing the Care City Team
We welcomed Vedika, Michael, Mags and Hannah, who brought new skills in design research, evaluation and innovation, strengthening our capacity to engage communities and drive impact.
9. Inclusive Fun with Power House Games
Partnering with Power2Inspire, we hosted one of London’s first Power House Games, bringing 60 young people together for inclusive sports. The event celebrated teamwork, broke down barriers, and created lasting memories.
